Unfortunately there is no such option due to the very specific design which needs to also work on all mobile devices. You may need to modify the template which displays the portfolio and display the additional information via PHP.

if you want to achieve design of contact page as you can see on our demo preview,
you have to use page template with name “Contact Page” for your contact page.

And regarding submit button, we see this button at the bottom of form:
