2. The URL of the plugin you have purchased
3. Where exactly did you put the shortcode? How did you get it into the page?
4. Are there any JavaScript or PHP error?
5. What shortcode have you used?

Well after viewing the page it seems there is JavaScript Error:
error_caclculator

Is the problem also present if you use:
[CP_CALCULATED_FIELDS id="5"]

If no then please check why the 6th one generates error.
If the problem is present please try to deactivate all 3rd party plugins as all is working fine with Boldial theme on our testing page.

Please try to deactivate all 3rd party plugins and check if the mentioned plugin works fine. If yes, activate each of the deactivated plugins one by one to see which one was causing the problem.

Also consider re-installing the “Calculated Fields Form” Plugin as something might have gone wrong during the installation.

Also please try to switch to the default WordPress theme to see if the problem comes from the theme or something else. If you switch to TwentyFourteen theme is the shortcode working fine?
