Do you have the “Contact Form 7” Plugin installed and activated? It was one of the necessary steps during the installation as mentioned in the documentation in the “First Steps” section.

Please have a look in the documentation and mainly step 3a in “First Steps” section.
3a. Install “IshYoBoy Boldial Assets” & “Contact Form 7” plugins

Once activated, make sure to edit the page where the contact form is displayed and set which contact form it should use.
