Unable to add images to portfolio items

For some reason I’m now unable to add any more images to my portfolio items. I tried reseting my cache, tried using different browsers, nothing works.

It was fine before now I go to add an item from the media library, I select it and hit “add image” but it just doesn’t show up. I’ve even tried it for several different portfolio items, doesn’t work for any of them.

Driving me crazy.
